A resolved comment does not show whether the writer accepted the request, narrowed the claim, supplied evidence, or removed the passage.

For comments affecting facts, scope, or identity, record the request, response, disposition, and approver. Keep consequential decisions in the release record.

Link the log to the reviewed version and identify changed passages. A brief entry is enough when it explains what happened and who accepted it.
Who decides
The writer should not mark disputed evidence approved. Unresolved high-risk comments remain with the editor or specialist.
